Conference Registration Forms

Unfortunately, you will still need to send the form by US mail.

**Very Important** Please check each activity that the participant plans on attending. These activities are listed under the name section on each form. All registration must be sent in no later than October 20. No late registration will be accepted. Also, a $30 registration fee is required with the registration of each person attending the Conference. If a contestant is registered for more than one contest event (i.e., Poultry Judging and Avian Bowl) it is still just a one time registration fee of $30.
